Enter the following information on the back of Form 3602-R (for regular rates) or  3602-N (for nonprofit rates).  Use Part B of the form (Non-Automation Compatible Letter) for letter-size mailings, or use Part C of the form (Other Nonletter) for flat-size mailings.
